Eligibility
To access the benefits of the Chronic Disease Programs, a physician (or community health nurse, in communities without a resident physician) must apply on behalf of the patient. Applications should be made before making purchases, but in some circumstances purchase prior to approval will be reimbursed.
Coverage
Provides assistance for medically-necessary prescription drugs, medical and surgical supplies, food supplements, and prostheses. These products must be recommended by a medical practitioner licensed to practice in the Yukon. Ostomy supplies are included in the “medical supplies” designation.
If coverage can be supplied through an existing federal act or private/group insurance, patients should access coverage through those methods first. Any remaining costs may be covered by Yukon CDP. If products can be loaned from a hospital or the Canadian Red Cross, the costs of buying these products will not be covered.
Deductible
Program recipients are required to pay the first $250 of eligible costs each year, to a maximum of $500/family.
Supply Limits
Neither Pharmacare nor CDP will pay for more than a three month supply of benefits at a time. There must be at least 75 days between the dispensing of a 3 month supply.
Exception Drug Status
Physicians submit an application for EDS on behalf of their patients. Initially, only 30 days coverage will be granted.
